We assign a case manager who will coordinate an individual treatment plan for each member. Dependent on the needs of the individual, services will then be acquired from other ADvantage contractors, which may include:

• Adult day health care
• Consumer directed personal assistance services & supports
• Home-delivered meals
• Safety equipment
• Hospice
• Occupational therapy
• Personal care
• Physical therapy
• Prescription drugs
• Specialized equipment & supplies
• Speech therapy
• Supported restorative assistance
